Medical courier Bryan is wrapping up a long shift when his dispatcher offers him double his standard fee to run a mysterious package to Raccoon General Hospital in Raccoon City, Colorado. Bryan takes the job, distracted by an unresolved phone call with his pregnant girlfriend Michelle. While driving down a deserted, snow-covered road near the Arklay Mountains, Bryan accidentally strikes a disoriented woman wandering across the asphalt.
Panicked, Bryan moves the unresponsive woman into his back seat to seek medical help. Arklay County Deputy Sheriff Wilson pulls him over, inspects the woman, and instructs Bryan to follow his patrol car to town. En route, the woman violently convulsing in the cruiser attacks Wilson, causing the police car to flip off the road and killing the deputy. Bryan manages to grab Wilson's service revolver, finding only a few rounds inside.
Bryan enters the foggy, barricaded streets of Raccoon City and reaches Raccoon General Hospital on foot. Inside, the building is dead silent, covered in blood, and strewn with wrecked equipment. Bryan quickly discovers that an experimental viral agent has escaped containment, transforming staff and patients into ravenous, mutating creatures. He encounters Umbrella scientists Dr. Carl and Dr. Paul, alongside enforcer Dr. Maxine, who are holed up in a secure upper-level research station. They explain that the package Bryan carries contains the stabilizing reagent required to finalize a cure.
To deliver the package, Bryan must climb through multiple quarantined floors and solve mechanical bypass locks after elevator power is cut. Along the way, he faces off against acid-spewing infected and a giant, multi-bodied amalgam creature stalking the shafts. Bryan burns through his bullets, scavenges medical sprays, and navigates a horrifying maternity ward filled with mutating flesh sacs.
On the exterior skybridge linking the two hospital towers, the amalgam cornered Bryan one last time. Bryan fires his final bullet into an emergency release door, sending a horde of infected cascading onto the monster and pitching it over the ledge to its death.
Bryan stumbles into the secure research lab and hands over the courier package, but the physical toll catches up to him. During an earlier grapple, he was infected. As the scientists rush to mix the cure, Bryan loses control, screaming for help as his body mutates violently. The mutated Bryan slaughters the researchers in a feral frenzy. The last standing scientist attempts to inject the cure into Bryan, but drops the syringe through a grate into the dark abyss below. Outside, National Guard helicopters circle the shattered hospital, locking spotlights onto the fully mutated Bryan as he lets out a monstrous roar.
